Begin here. Titrate conservatively based on response.
Standard Range
Maintenance/Minor Injury: 0.5-1mg (250-500mcg each) daily. Major Injury: 1-2mg (0.5-1mg each) daily.
Titration Path
Increase to 1-2mg for major injury.
